Horizon House offers a broad range of housing options for adults with intellectual and developmental disabilities. We foster maximizing independence while providing assistance equally needed to individuals. Residential options include Customs Residential Living, Community Residential Rehabilitation, Supported Living, and Life Sharing.

Community Residential Living

Community residential living is provided in homes accommodating up to four individuals with in-dwelling back up available 24/seven. Homes are licensed and monitored by Horizon Firm staff including quality improvement specialists. Using a multidisciplinary approach, staff members are involved in each resident'due south specific goals and outcomes. Every consideration is fabricated to provide for an individual's preferences, including home decor, leisure time, repast planning, recreation, and therapeutic activities. Family unit participation is encouraged and welcomed.

Community Residential Rehabilitation – Forensic Services

Adult males with maladaptive psycho-sexual behaviors are assisted in developing skills and positive attitudes necessary to alive and work successfully in the community while living in an intensively supervised environment. Individual and grouping treatment are provided to teach individuals to monitor and self-manage thoughts and behaviors. Trained staff foster grouping involvement, positive peer interaction, values description, problem-solving skills, health, nutrition, drug and alcohol educational activity, interpersonal effectiveness, and competitive employment supports.

Life Sharing

Life Sharing  (Family Living) provides adults with intellectual and developmental disabilities with a matched family to live with and share everyday experiences. Supports are individualized and include everyone in the home. Life Sharing provides individuals the opportunity to be part of a family and participate in customs life. Our staff works in partnership with matched families to ensure the individual's needs are met. Individuals who participate in life sharing savour a long – term back up organisation.

Our life sharing residential arrangements are express to homes with no more than two individuals with disabilities living in the aforementioned dwelling and who are not family members/relatives of the host family unit members.

Supported Living

Supported Living is available for adults with intellectual disabilities who are able to live safely in the community with limited supervision. Staff is available to assist and back up individuals with budgeting, health maintenance, employment and activities of daily living. Staff encourages and educates individuals to increase self-confidence and independent living.
